Exploration of right heart

Sonnia Posted Thu 21st of July, 2016 10:56:10 AM
Can we bill for the exploration of the right heart if no thrombus was found during a pulmonary embolectomy? If so, should I use 33310-33315?

Though the codes for exploration of the heart are correct, the details of this case would decide whether they can be reported here. These codes usually represent exploration after injury due to external trauma or surgery of the heart, therefore please provide some more details of the case for us to get more clarity for answering the first part of your question. Thank you.

The right heart was opened to ensure there was no thrombus in transit in either the atrium, ventricle, or the ventricular outflow tract. There was no evidence of thrombus in these areas.

As per our understanding, the provider was performing pulmonary embolectomy during which it was discovered that there was no thrombus. Since pulmonary embolectomy is itself an elaborate procedure (please see the attached link for more details under the section "How Procedure is Performed") with a specific code, and the provider discovered the absence of thrombus during this procedure itself, the exploration need not be coded separately in this case. Thank you. http://www.mdguidelines.com/embolectomy-pulmonary
