when under the K 92.2 it says excludes 1. peptic ulcer with Hemorrhage (k25.-K28) does that mean only the ones with Hemorrhage only or does that mean all the codes since those codes include without hemorrhage

Hi Sherry,

 When your documentation mentions peptic ulcer with hemorrhage or without hemorrhage we will go for code range (K27-K27.9). (it means it includes both with and without hemorrhage codes.)

Excludes means “Not coded here”.

However, when documentation mentions gastrointestinal hemorrhage, then here it means patient suffers from GI hemorrhage but provider does not mention type of hemorrhage such as whether hematemesis (blood in vomitus) or melena (blood in stool), so, we will use K92.2 (gastrointestinal hemorrhage, unspecified).
