excision spinous process

What would the cpt codes be for removal of inner spinous ligaments (excision spinous process) CPT code 22102 is what the surgeon is doing but not for intrinsic bony lesion, can I use this code if not for intrinsic bony lesion? He is also doing a posterior fusion at L3-4, L4-5 with application of interspinous plate, which would be 22612, but he said that he is only putting the plate on one level, so would I use 22840? Please help I need to get his surgery pre-cert and need to use the correct codes! Thanks

There is no CPT code for removal of inner spinous ligament since it is considered a part of the major surgical procedure that was performed on the billed DOS. Answer to the second part of your question: You should bill CPT code 22612 and 22614 for posterior fusion performed at L3-L4 and L4-L5 level. Also, you would code CPT code 22840 if Posterior non-segmental instrumentation is performed.
