I need some help with this surgery coding.
Patient is a 1 yr old boy with congenital malformation of left facial skin. The left side of the face was steriley prepped and draped. 1% lidicaine with epninephrine was infiltrated into the are of the base of the large preauricular cyts. They were excised with pickups and 15 blade scalpel. In the are of the tragus we preserved the tragel cartilage and draped the existing skin over the cartilage over the cartilage to try to reform tragus. Interrupted 3-0 Vicryl sutures were used for the primary closure. Baciticin was applied.

The appropriate code for the above scenario would be 11200. Report 11200 if mentions of skin tags up to and including 15 lesions; each additional 10 lesions, you can report additionally +11201.

What about the closure and draping of the tragus, wouldn't we be able to code and bill for this?

Tragus is a small pointed eminence of the external ear, situated in front of the concha, and projecting backward over the meatus. I don't think there is specific CPT for reconstruction of the tragus. Of course, there is a code that can be used for reconstruction of tragus (69110), but this code says excision of full thickness section of the external ear with simple repair and it's not appropriate in this scenario. The below link says concha and tragus reconstruction (3rd stage), it can be coded as 69110.
