Excision of mass of nail bed of right index finger

Cheryl Posted Thu 29th of August, 2019 14:02:23 PM
What CPT code would be best used for the following: The surgeon elevated the nail plate to expose the underlying mass on the nail bed. The entire mass was excised down to the periosteum of the distal phalanx. The area was irrigated and closed with suture. The nail plate was secured back on with suture. Path report states uncertain behavior.

Hello Cheryl,

Thank you for your question.

As per the above provided documentation, CPT® 26115 (Excision, tumor or vascular malformation, soft tissue of hand or finger, subcutaneous; less than 1.5 cm) would be appropriate. Also add modifier F6 (Right hand, second digit) indicating the anatomical site. i.e. 26115-F6

Note: This is a size-based code, kindly check your detailed medical documentation before reporting the code.

Use this code (26115) when the resection size is under 1.5 cm Or, code 26111 (when the resection size is 1.5 cm or greater.

Hope that helps!
