What is correct way to code excision and repair done to the right post shoulder superior 2.9 cm and right post shoulder inferior 2.5 cm?
11406 and 12032 or 11403, 11403-59, 12032

refer prior biopsy
moderate
anasthesia: 10 cc lidocaine w/ epinephrine
4 mm margins
no complications
minimal blood loss
keflex x 5 days
suture 4-0 prolene 4-0 vicryl
R post shoulder inf: 2.5
R post shoulder sup: 2.9
final defect: 5.4 cm

1) Excision of R post shoulder sup: 2.9 plus 4 mm margin on each side = 0.4 + 2.9 + 0.4 = 3.7 cm (Code 11404)
2) Excision of R post shoulder inf: 2.5 plus 4 mm margin on each side = 0.4 + 2.5 + 0.4 = 3.3 cm (Code 11404-59)

Intermediate repair of final defect of size 5.4 cm (Code 12032).
