Excision closure

Hi, after skin cancer has been excised from patient's back, would placing 4-0 Monosyn sutures in the deep dermal fat junction x 3 and in simple interrupted manner in the epidermis qualify for intermediate excision closure or this would be considered a simple closure, which is included in the excision itself?

This qualifies for a simple repair only which is included in the excision itself. A simple repair includes trimming or removing fatty tissue and cleaning the wound. You would refer to codes 12001-12021 to choose the specific code by the length of the wound. Be aware that, according to CPT, a single-layer closure of a heavily contaminated wound that needs extensive cleaning would elevate it to an intermediate repair.
