WHAT CODE WOULD BE BEST TO USE FOR THE FOLLOWING PROCEDURE DECRIPTION? AN OVOID INCISION WAS MADE OVERLYING THE PURPLE LESION RADICULAR TO THE VERMILLION BORDER. THIS INCLUDED SOME OF THE OVERLYING ATTENUATED SKIN. SKIN HOOK RETRACTION CURVED IRIS SCISSORS WERE USED TO DISSECT DOWN TO THE SUBCUTANEOUS PLANE. NO DEFINITIVE CYST ALL WAS ENCOUNTERED AND CAREFUL DISSECTION WAS PERFORMED AROUND THE AREA OF THE SCAR AND INFLAMMATORY TISSUE. THIS WAS EXCISED DOWN TO THE LEVEL OF THE UNDERLYING ORBICULARIS MUSCLE. THE LSION WAS EXCISED AND SUBMITTED FOR PERMANENT PATHOLOGY. UNDERMINING WAS PERFORMED AT THE DEEP TISSUE PLANE ABOVE MUSCLE AND HEMOSTASIS CONFIRMED WITH NEEDLE TIP ELECTROCAUTERY. DEEP LAYER CLOSURE WAS PERFORMED WITH INTERRUPTED 5.0 MONOCRYL SUTURES STARTING WITH VERMILLION BORDER REAPPROXIMATION AND THEN SUPERIOR AND INFERIOR TO THIS. SKIN CLOSURE WAS PERFORMED WITH INTERRUPTED 6-0 NYLON SUTURES ON THE SKIN AND 5.0 PLAIN GUT SUTURES ON THE RED LIP.

Hi Victoria,

The appropriate code for the above procedure will be from the code range 11440 to 11446 depending upon excised diameter. Complex or layered closure is reported separately, 13151. However, 13151 bundles to 11440 but we will overrule this by adding modifier 59 to 13151. Hope it helps.
