As per above scenario, if only EXCISIONAL BIOPSY is performed, then the appropriate CPT code would be 56605 {Biopsy of vulva or perineum (separate procedure); 1 lesion} which is designated as a separate procedure, so you should not report it when the provider performs the biopsy as part of a larger, related service. You should report this code only for excision and biopsy of the first lesion. For removal of each additional lesion, use +56606 {Biopsy of vulva or perineum, each separate additional lesion}. These codes describe excising the lesion for biopsy, and therefore would be the codes of choice if this is the purpose of the procedure rather than therapeutic removal of the lesions.

In order to report therapeutic removal of the vulvar lesions, see CPT code range series 11420 to 11426 and 11620 to 11626 which will be chosen as per documentation depending on the nature of the lesion (benign or malignant) and size of the lesion. If a specimen is transported to an outside laboratory, report 99000 for conveyance.
