When billing for '93272', would the date on the claim need to be the date it was actually ordered or the date the patient hooks it up? The patient receives it thru the mail from a outside company.

Check to see if your payer has published specific guidance as preferences can vary regarding whether to use date of hook-up or date of completion, especially as this is the interpretation code.
