Evacuation of intracerebral and subdural hematoma

The neurosurgeon performed a craniectomy for evacuation of subdural and intrecerebral hemorrage, in addition a duraplasty used temporal muscle and Duragen. What is an appropriate CPT code for this procedure?

For craniectomy done for evacuation of subdural hemorrhage, use code 61312, Craniectomy or craniotomy for evacuation of hematoma, supratentorial; extradural or subdural.

For craniectomy done for evacuation of intracerebral hemorrhage, use code 61313, Craniectomy or craniotomy for evacuation of hematoma, supratentorial; intracerebral.

As per CCI edit, code 61312 is included in code 61313. You may use both the codes only when the provider performed both procedures through separate incisions. Use modifier 59, distinct procedural service with code 61312.

Also, duraplasty is normally included in the craniectomy code as part of the closure and not separately reportable.
