Patient was seen in office for hysteroscopic placement of essure. 10 days later she had it removed in an outpatient setting because she had concerns related to possible nickel sensitivity. The sensitivity has not been verified. Would I use the unlisted code '58579' with dx of v2512? Are there any other dx codes that would be appropriate in this situation?

If the Essure was removed with a hysteroscope then code 58562 or else 57415, along with this use 996.35 and E947.9

Thank you for your help. The dx code '996.35' is not listed in the 2011 icd-9 manual and comes up unknown on the coding software that we have in place.

It must be a typo instead of 996.35 it is 996.65.
