ESI coding

We have two different groups of physicians in our hospital who do ESI's (Epidural Steroid Injections). One group are our radiologists who perform them in the radiology department and the other are the Pain mgmt physicians who can perform them either in our radiology department or in the O.R.

My question is, what should we be charging in all three situations?
Charges CODES
Injection Charge 62311
Procedure 77003
contrast 180/20

Can we still charge both the Injection and the procedure if a non-radiologist is performing them. And if performed in the OR, we are currently only charging fluoro <hour or fluoro >hour

Can be performed by Radiologist who will bill 62311 and 77003-26
When performed by Pain management physician, they also bill in the same manner. This is what we witness in many coding companies.
Only fluoroscopy is billed in the OR when only Film is obtained as billing is based on the documentation.
We should also examine differing opinions and logic to come to correct conclusion.
