As per the above-mentioned documentation, the appropriate CPT code would be 61624 [Transcatheter permanent occlusion or embolization (eg, for tumor destruction, to achieve hemostasis, to occlude a vascular malformation), percutaneous, any method; central nervous system (intracranial, spinal cord)]. Since, your doctor has placed two additional pipeline embolization devices, so you append modifier 22 (Increased procedural service) with the CPT 61624. For additional two pipeline embolization devices supply code C1884 (Embolization protective system) can be billed if payer allowed.

Also attach copy of operative report along with bill to justify the medical necessity.
