Just wondering on these rules....If the MD documents "see HPI for the Exam"...Does that count? I can't find any reference to support me telling the MD that this doesn't work.

No, you cannot see HPI for the Exam. As HPI consists of the findings which are told by the patient or the attendant, however, examination part is performed by the physician on the patient.
