EEG Monitoring and Video Recording_CPT Code: 95951-26

This is a three part questions for EEG video monitoring, code- 95951-26 (Professional Component only). We would like to know the correct way to bill for the following EEG video monitoring scenario:
1) EEG monitoring starts on 10/20 from 10 pm to 12 am (total 2 hrs.), 10/21 from 12am to 12am (total 24 hrs.) and 10/23 12am to 12pm(total 12 hrs). Total hours for the three days is (38 hrs).

2) If the monitoring is one hour or less, can we bill 99251-26-52 or is there a time limit.

3) The monitoring is based on each 24 hours. If the monitoring start on one calendar day and end on the next, is it ok to use the date that the EEG ended as date of service for both days?
If available, please provide referral for guidelines.

For day 10/20 use CPT 95951-26-52. For day 10/21 and 10/23 use CPT 95951 with 26 modifier only. If, it less than 24 hours, use modifier 52 irrespective of the date change. The monitoring code is hourly based, not on the basis of days, so while coding check the number of hours.
