E &M codes 99291 & 99292

Is there anyway that these two codes can get paid--I have several claims and it seems they get denied most of the time---even when I've sent documentation. Do they need to have times so that they can see how long they were with the patient or is there a modifier I can use. We billed the following codes 99291 - 99292 -99292 57 -13101 & 12034 with the dx of 876.0 & 875.1 .

Both codes 13101 & 12034 have a 10 day global period. Are you billing this all under the same provider?

Yes they are the under the same provider ; however the 12034 has mod 59 and they got paid. it's the the 99291 & 99292 the 99292 with mod 57 got paid as well
