dx codes with an annual exam

When billing an annual exam -z01.411 or z01.419- is it necessary to include separate codes for encounter for screening breast exam, encounter for pap smear of cervix, screening for osteoporosis, surveillance of ocp, etc. I thought these would be included in the annual exam code. My doctors say the insurance company needs to know these were done for data collection. Do I need to include these codes on the claim?

Yes, you need to include separate codes for encounter for screening breast exam, encounter for pap smear of cervix, screening for osteoporosis, surveillance of ocp, etc. along with annual exam z01.411 or z01.419.
