DX code for J0585 and Medicaid

I need help trying to figure out which code to use for chronic migraines. Medicaid will only allow the following dx codes and they are mostly for spasticity. Can somebody please help me with this ......333.6

Hi, Patricia, and thanks for the question.

The typical codes for chronic migraine according to ICD-9 come from 346.xx (migraine), including 346.7x (for chronic migraine without aura). After looking at all the codes you've listed, I'm stumped. Have you talked with one of your Medicaid representatives to be sure the list is complete? If you can let us know what state you're in, we might be able to help you pinpoint a better answer.

I am in Indiana and I did speak with some reps and they don't cover Botox for migraines....odd huh
