dural tear

K Posted Thu 27th of March, 2014 13:00:43 PM

Hello, our provider blocked a lumbar dural tear with a single piece of gelfoam. Would you code this at 62273?

I would not report 62273 for application of a piece of gelfoam to seal over an epidural tear. From my OR experience, the gelfoam is not inserted as a dural repair via an injection rather typically it is an open surgical procedure. If the dural repair is performed via the open surgical approach, the codes to look at include:
63707 Repair of dural/cerebrospinal fluid leak, not requiring laminectomy

63709 Repair of dural/cerebrospinal fluid leak or pseudomeningocele, with laminectomy
If the dura repair is performed during another spinal procedure, it typically is included as part of the surgical procedure and not separately billable.
