Dupuytren's release in palm/MCPJ

How would you code the following? Dxs are 1)dupuytren's contracture of palm at rays 3-5 and 2) 5th MCPJ contracture. Doctor performed palmar fasciectomy at rays 3-5 and releasing A1 pulley at 3,4, and 5 which decompressed each flexor tendons out to A2 pulley. At the 5th MCPJ, extension was improved by releasing the contracture and excising a nodule at the MCP. There was no diagnosis of trigger finger. Would you code 26121 for the entire case or 26123,26125x2 for each finger? Please give explanation for code chosen. Thanks!

Hi, You would use CPT code 26123 and 26125x2 for the above scenario. If a fasciectomy is performed to include the fingers, it would be appropriate to report 26123 code. Thanks.
