Dual Port Feeding Adaptor replacement

Tammy Posted Tue 07th of August, 2012 20:37:42 PM

Patient had a dual port feeding adaptor replaced in the office. This was done by the nurse. What code do I use for the adaptor? Also, can I bill '99211' and use a modifier 25 in conjunction with the port?
