Thanks for your question. Documentation for vaccine administration should include the date of service, manufacturer, lot number, vaccine type, name and title of the person who administered the vaccine, vaccine information statement with printed dates as well as the route, dose, and site. The physician should have orders in the patient's chart whether the physician is in the office at the time of the administration or not. See attached link for CDC guidelines on vaccine administration.
