Digital Hemoglobin for a school form

Hi,

We had a pediatric patient come in for a hemoglobin that was needed for a school form. The patient was seen with nurse only. We bill88738 for the digital hemoglobin -- what diagnosis code could we use? We don't think V20.2 is correct since the patient did not have a physical with this visit and has not had his yearly physical yet this year. V78.0 seems appropriate but it does not seem this code can be used as a primary or only diagnosis code. Please advise ..

Well, Hb check is for school form, which is an administrative purpose, ICD-9 V70.3 (Other general medical examination for administrative purposes) seems the appropriate to append with the record. Hope it helps!
