What is the difference between the codes 77002 and 77003? Is it location? Only 77003 is for spine and 77002 is for all other body areas? Or does it come down to if contrast was injected? If contrast is used then do 77003 and no contrast 77002?
The description of 77003 states Fluoroscopic guidance and localization of need or catheter tip for spine and paraspinous diagnostic or therapeutic injection procedures. Then it states (injection of contrast during fluoro is included in .......)So if using contrast does that change the meaning of 77003 guidance and localization and contrast injection or is there a different code for injection of contrast?

Thank you for your question. The injection of contrast is not included in the codes 77002/77003 (unless otherwise stated for 77003) These codes are Fluoroscopic Guidance only. 77003 includes localization of the needle or catheter and is used only for the spine and paraspinous area.
77002-Fluoroscopic guidance for needle placement (eg, biopsy, aspiration, injection, localization device)...This codes is for needle placement, not body area specific. This code does not include the injection procedure. This code is typically used with major joint injections.
77003- Fluoroscopic guidance and localization of needle or catheter tip for spine or paraspinous diagnostic or therapeutic injection procedures (epidural or subarachnoid) ...(Injection of contrast during fluoroscopic guidance and localization  is included in 22526, 22527, 27096, 62263, 62264, 62267, 62270-62282, 62310-62319) This code is typically used for epidural procedures or nerve blocks

So if a provider does an steroid epidural injection 62311 under fluoro for guidance and injecting contrast to confirm location do you bill 77003 with 62311?

Yes, that would be correct. I checked the CCI edits to confirm and both codes may be billed together.
