Diagnostic Laparoscopy and Removal of Reordered Lap-Band with Repair Gastrotomy

Please provide me the CPT coding for following scenario:
Diagnosis: Small-bowel obstruction with eroded Lap-Band

Procedures: Diagnostic Laparoscopy, Removal of reordered Lap-Band & Repair of gastrotomy.

Brief explanation of the procedure: Upon entering the abdomen, pt found to have an extremely dilated and distended small bowel and it was secondary to the Lap-Band tubing which had actually wrapped around the small bowel and causing obstruction. Doctor tried to undo the kink, however it was extremely difficult. At this point doctor elected to cut the tubing and relieve the obstruction, so that doctor could proceed with running the bowel.

After following the tubing it was elected to visualize the band. Following the proximal end of the tubing lead to the Lap-band itself, after taking the pseudocapsule out, realized that the band has actually been eroded, hence the eroded band was removed after being cut. Approximately a 3cm gastrotomy was then encountered which was repaired.
