diagnosis code to use when the parent doesn't know how the child hurt themselves

We had an 18 mth come in for a visit at our pediatric office and they had a bruise on their chest. The parents have no idea how the child got the bruise. I put the icd 10 diagnosis code of S20.212A, but I don't know what else to put with it for how it happened or where they were because the parents don't know. Is there a code or unknown how & where?

Well, ICD S20.212A is appropriate to code. For unspecified accident there was a ICD-9 code E928.9, the crosswalk of ICD-10 for code E928.9 is X58.XXXA. So, you can use X58.XXXA code as secondary along with S20.212A. Hope it helps!
