Are we coding this procedures properly?

Dysplastic Nevus: 238.2 or should it be 216.5

Excision right upper chest 2.6 cm and L paraspinal back 3.2 cm final defect: 11404, 11403, 12032

Dysplastic Nevus- 216.5, Benign neoplasm of skin of trunk except scrotum would be more appropriate.

Excision right upper chest 2.6 cm- 11403 is correct.
Excision left paraspinal back 3.2 cm- 11404 is correct.

12032 represents 'Repair, intermediate, wounds of scalp, axillae, trunk and/or extremities (excluding hands and feet); 2.6 cm to 7.5 cm'.

You may use 12032 if the provider has performed intermediate repair and the combined length of repair at both sites at back is falling in range of 2.6 to 7.5 cm.
