CPT 99203-25 11100 40490, 21550 was billed do we add a modifier 59 to 11100?

Thanks for your question. According to CCI, when billing these four codes together you would append the modifier 59 (distinct procedural service) to 11100. 11100; Biopsy of skin, subcutaneous tissue and/or mucous membrane (including simple closure), unless otherwise listed; single lesion is a column 2 code for 40490; Biopsy of lip.
