We are billing 17000 DX 702.0, 17003 dx 702.0 and 11900 59 dx 701.4 and we can not get the 11900 paid they are always denied as bundled even appealed with backup?

For the same site where destruction is taking place, you cannot code for injection for the same lesion. You can only code if its a different lesion.This is the reason of denial. Provide sufficent evidence/documentation if it is a differnt lesion to separately report it with a 59 modifier.
