Debridement of tympanic membrane edge

The patient had a tympanic membrane perforation. The doctor noted squamous debris growing on the undersurface of the tympanic membrane. He used suction and a Rosen pick to removal all visible squamous debris under the operating microscope.

Would 69450 be appropriate? If not, unlisted?

Code 69450 (tympanolysis, transcanal) would apply in this case. Coders Desk Reference notes that the otolaryngologist treats a lesion or other irritation to the tympanic membrane. Using an operating microscope, adhesions (or scar tissue, in this case) are removed from the surface of the eardrum.

The use of the operating microscope is included in the code and should not be billed separately.
