Debridement arthroplasty , talocalcaneal joint

Dx Arthritis, talocalcaneal joint & soft tissue mass dorsolateral ankle. Path report states ganglion cyst and bursitis.

The mass was excised followed by a synovectomy of the peroneus brevis and longus tendons. I am thinking about using codes 27630 and 27681 though they do hit an edit. I see only one incision therefore I will not bill them both. My question is, because the doc states "both the talocalcaneal and ankle joint in the area were shown to be arthrtic with periarticular osteophytes" a bone decompression and debridementy arthroplasty in the area of the mass was done.

I am not sure if the debridement should be billed in addition to the excision of the mass. (same area) (same incision).

What are your thoughts?

Not at all. I agree with your approach only 27630 will be coded here and debridement will be inclusive to it. Its a same incision which again confirms no additional reporting of debridement codes.
