Cysto with Bladder Biospy vs. Resection

When Urologist performs cystourethroscopy and takes a bladder "biopsy" then CPT code 52204 would be used?

If the Urologist resects the lesion/tumor then it would be 52234, 52234, or 52240 depending on size?

CPT 52224 is used when fulguration is performed in one area and a biopsy in another (separate) area?

3)CPT 52224 is used when fulguration is performed in one area and a biopsy in another (separate) area? - Doesn't seems to be correct. It can be either ways. The provider may perform fulguration and biopsy in same area or he may perform fulguration in one area and a biopsy in another (separate) area.
