Please clarify whether it's CT-guided aspiration (possibly 62268, 77012 assuming it's not included in another service at same encounter) or whether it's surgical decompression.

The injection of the facet joint demonstrated communication with the synovial cyst noted within the spinal canal at this level. High pressure was applied to the cyst using
5cc of Isovue-300M. The cyst decompressed at the end of the 5cc injection. Visualization of the extension of the contrast into the epidural space was noted. However, to confirm decompression of the cyst, direct access to the cyst was obtained via the right paraspinal L4-L5 interlaminar approach. The injection of the cyst demonstrated leakage of the contrast into the epidural space.

It is unclear to me with the limited vignette provided whether this was completely a percutaneous procedure done with a needle or a combination of open and percutaneous techniques. I would need more information. Based on what you sent, this is most likely reported with an unlisted code.
