How would you code a case where a 2nd ventricular (EVD) catheter was placed on the right lateral ventricle and tunneled to the left side to join the left ventriculoatrial shunt?

Use CPT code 61210 with add on code 62160.

If you are replacing the existing ventricle catheter with a new EVD that connects to the shunt on the other side; wouldn't you code 62225?

Sorry, I meant if you are connecting a right Bactiseal EVD to the the left ventricular catheter with a t-connector; would you charge 62225?

CPT code 61210 is for implanting ventricular catheter for first time placement.
CPT code 62225 is for replacement or irrigation of ventricular catheter.
You can use the code as per your scenario.
