Craniotomy Help

How would you code for a right parietal craniotomy for bx of
leptomeninges and brain?

I believe you'll need to use an unlisted code (64999). There is a brain biopsy code through burr hole (61140), but that doesn't fit. An expert explained to me that the closest craniotomy code is supratentorial exploratory (61304), but close isn't good enough. The meningeal biopsy is considered incidental to brain biopsy. If a stereotactic technique was used, look to 61750 or 617151. But since you didn't mention that (and I believe it's normally performed through burr hole), unlisted looks like the most appropriate option.
