Cranioplasty with a small dural tear repair.

Would a small dural tear repair be considered a reparative brain surgery?In a situation where a cranioplasty 5cm was done with cranial flap obtain from the abdominal pocket for a cranial defect; would you code this as 62146 and 62148; or since there was a small repair of a dural tear; would you code 62145 and 62148?

62146 is appropriate code for the given scenario, and it is considered a small repair of the dural tear part of the surgical package. If the neurosurgeon repaired a more significant tear, however, you can append modifier 22 (Unusual procedural services) to the appropriate procedure code. For example, if he had to perform a repair requiring additional exposure and/or graft application, you should use modifier 22. And use 62148 as add-on code when incision and retrieval of subcutaneous cranial bone graft for cranioplasty is being performed.
