CPT for muscle reinneration

Arm was amputated at the forearm level. The median and ulnar nerves were neurolysed. The proximal portion of the ulnar motor branch was repaired to the distal portion of FDS motor branch. Then the distal aspect of the median nerve was approximated to the remaining distal aspect of the ulnar nerve. Any suggestion for nerve portion of this procedure?

As per the above documentation Cpt codes 64718 (Neuroplasty and/or transposition; ulnar nerve at elbow) and 64721 (Neuroplasty and/or transposition; median nerve at carpal tunnel) seems appropriate.
