The patient was already placed on cardiopulmonary bypass and had undergone superior vena cava cannulation and caval snares and had already undergone excision of the right and left ventricles. The pericardial flap was then removed from the right atriotomy, thereby revealing the left atriotomy right above the right pulmonary veins. This portion of the left atrium was closed with a patch of CorMatrix with a running 4-0 Prolene suture. We then inspected the left interatrial septum and then took down the suture line of the lateral wall of the right atrium that formed the roof of the systemic venous pathway thereby exposing the orifices of the superior and inferior vena cava. The inferior vena cava seemed somewhat narrowed at its orifice. The atrial septal flap was then taken down and then sutured in its orthotopic position to close the atrial septum. We then closed the right atriotomy and this was done with a running 4-0 Prolene suture. Inferiorly, the inferior vena cava was augmented with a patch of CorMatrix and this was included in the atriotomy suture line. The coronary sinus was oversewn. At this juncture, the implant of the total artificial heart resumed and at the end of the case we decannulated and we repaired the left femoral artery primarily with interrupted 6-0 Prolene sutures and the pursestring suture around the left femoral vein cannula was tied down with good hemostasis. That groin incision was closed with absorbable sutures in layers. The skin was closed with a running 3-0 nylon suture. After this sternotomy incision was closed, the pacemaker generator site was reopened and the pacemaker generator was explanted and the wound irrigated. The incision was then closed in layers with absorbable sutures in standard fashion. There were no immediate complications.

There is no listed code for the above procedure. You will have to use unlisted code, 33999, Unlisted procedure, cardiac surgery.
