cpt codes 49651 and 49650

are these codes normally paid separately? I reecived a claim form indicating bilateral inguinal hernia and both of these codes were attached. secondary question, does a bilateral count as two codes for paymetn under CMS?

CPT code 49650 represents Initial inguinal hernia.
CPT code 49651 represents Recurrent inguinal hernia.

These codes can be billed together with 59 modifier in circumstances that 1 side inguinal hernia is of initial and other side is recurrent in nature.

Also, Bilateral modifier -50 is applicable on CPT code 49650 and 49651.
