My doctor has given me a billing slip with laparoscopic parastomal hernia repair; lysis of adhesions; lap hern repair with mest;and finally Para colost hernia. I would appreciate and all replys.

49659: Unlisted laparoscopy procedure, hernioplasty, herniorrhaphy, herniotomy
If the surgeon did a revision of the colostomy with repair of a paracolostomy hernia, then use code 44346.
If two separate and distinct hernias were repaired (such as parastomal and ventral), then it is appropriate to also report code 49560 with a mltiple procedure modifier -51. If mesh was used for the ventral hernia repair, use 49568 as an add on code.
Lysis of adhesions is always considered incidental to above procedures, but it involves significantly extended time and effort, then you can use modifier 22 with CPT 49560 and also submit medical records while filing.
