How would you code L4 & L5 laminectomy, L4-5 diskectomy, L4-5 interbody fusion, L4-5 placement of interbody prosthesis, posterolateral instrumentation L4 through S1, posterolateral arthrodesis, L4-5 and L5-S1

Hi,

 

As per the given documentation, the appropriate codes are as follows:

22633 -  laminectomy,  diskectomy, interbody fusion and posterolateral arthrodesis at L4-L5 level

22614 - posterolateral arthrodesis at L5-S1 level

63047 – for decompression at L4-L5

63048 - for decompression at L5-S1

22842 - Instrumentation

22853- L4-5 placement of interbody prosthesis

 

Please check operative note for decompression. If decompression is performing at only one level, then only CPT 63047 will be coded. If physician performing decompression at two levels than 63048 will also be coded.

That's what I came up with but 63047 is bumping up w/ code 22633 per CCI EDITS. Do I use a modifier 59 or 51?

Hi,

Yes, you have to use modifier 59 or 51 with 63047 depending on payer preference for modifiers.
