The appropriate code to bill for removal of epicardial pacemaker and dual leads via sternotomy will be CPT 33237.

but 33237 is via thoracotomy this was removed through reopening of sternal incision would 33233 and 33235? Thank you

There are several different types of thoracotomy that can be performed, depending on indication for surgery and condition being treated. For example:

Median thoracotomy: Incision through sternum to gain access to chest to reach heart.

posterolateral thoracotomy: Insicion through ribs to reach lungs.

There are many more approches to reach chest.

So if approach is sternotomy, broadly we can say median thoracotomy and bill CPT 33237. Hope it helps.
