CPT code for lap paraesophageal hernia

What would be the appropriate CPT code for laparoscopic repair of incarcerated paraesophageal hernia with Nissen fundoplication and gastropexy. I can only find 43281, 43282. Can I also charge for the gastropexy and if so, what is the CPT code

There is no code for the procedures you have described. You will have to go with the unlisted procedure code to report the laparoscopic Gastropexy. As suggested, 43659 (Unlisted laparoscopy procedure, stomach) is the proper code to report. Please provide documentation along with billing to insurance company, if your documentation support the procedure.

What about the laparoscopic incarcerated paraesophageal hernia repair? should I just bill with CPT code 43281 (done without a mesh) or go unlisted 43289?

If done without a mesh, bill CPT 43281. But if Mesh used, bill CPT 43282.
