CPT code for DDD-ICD replacement, lead extraction and DFT

Patient presents for replacement of his DDD-ICD generator at ERI. The existing RV defibrillator lead is not functioning well, so it is removed by transvenous lead extraction using a Laser sheath, in the Hybrid room. A new RV defibrillator lead is inserted. The old DDD-ICD pulse generator is removed and a new DDD-ICD pulse generator is inserted, and connected it to the new RV defibrillator lead and existing RA lead. Defibrillation threshold is done.
Which proper CPT code should be included in the claim.
I was suggested: 33249, 33244 and 93641-26. Should 93640 also be included?

I think the codes you suggested are correct. 33249, 33244 and 93641-26. Code 93640 is reported when the pacing cardioverter-defibrillator electrode leads are implanted during insertion but are not connected to the pulse generator. When a pulse generator is connected and the entire system is tested, then code 93641 is reported.
