CPT Code for Curettage of Perirectal Abscess

Procedure performed: colonoscopy, destruction of perianal fistula, curettage of perirectal abscess. What is the correct coding for curettage of perirectal abscess? Would it be unlisted CPT: 46999? Or is it bundled with destruction of perianal fistula? All the other codes are either for I&D or curettage of anal fissure (not abscess).

Please read the documentation carefully because perirectal abscess may be developed as a result of perianal fistula and there may not be a need to report curettage of perirectal abscess separately, which is considered as an integral part of destruction/correction of perianal fistula.

There is no CPT code for Curettage of perirectal abscess. Code 46999 is for unlisted procedure, anus, so you cannot use this code for perirectal abscess. Instead you may use 45999 (Unlisted procedure, rectum).

If there is mention of incision in the documentation, then you may report code 46040 (Incision and drainage of ischiorectal and/or perirectal abscess (separate procedure)).
