I NEED TO APPEAL CPT CODE 67040, IT WAS BILLED WITH 67042 AS A PRIMARY PROCEDURE,DX: VITREOUST HEMORRHAGE AND PROLIFERATIVE DIABETIC RETINOPATHY, THE INSURANCE IS INCLUDING 67040 WITH THE PRIMARY PROCEDURE CODE, THE ENDOLASER 67040 WAS GIVEN TO THE ELEVATION OF VITREOMACULAR TRACTION.

Per the CCI edit review, 67040 and 67042 may be coded together if the appropriate modifier was used. 67040 would need to be appended with modifier -59.
