When using cpt code 64910 allograft for nerves in surgery center setting(ASC) we are treating 6 nerves in the spine for example Treating Lumbar 2,3,4 bilaterally Six injections around nerves How do you bill it to Medicare and which modifiers do you use ect?

The MUE for code is 3 and 50 modifier is not allowed with code 64910.

So, you need to bill 3 units of 64910 for L2, L3 and L4 nerves. Bilateral payment is not alloweed with this code.
