Does the Physician have to on the premises to bill CPT 97140 (Manual Therapy) when being performed by a licensed massage therapist?

CPT 97140 says direct contact of the health care provider with the patient and "manual" therapy requires that providers utilize their hands in order to provide these techniques. So definitely yes, the physician have to be on the premises in order to qualify 97140.
