When billing a 76705 and physician performs ultrasound and interprets, is it a requirement to bill both a 26 and TC modifier? If it is required, is the proper way to bill this with one line with both modifiers listed or two lines with a 26 and TC listed separately.

When the physician performs the entire service of 76705, the service is considered global, therefore you do not need two separate lines. Modifier 26 and TC are used when the service needs to be separated out (when the physician only provides part of the service). In your case 76705 can be billed on one line, for the entire service, no modifier necessary.
